What Is an Aftercare Program?

Aftercare programs are dedicated to increasing the chances of long-term sobriety within recovering clients. The right motivation and adequate resources can go a long way in helping people to stay clean as they walk the road of recovery.

This type of program should be baked into any successful recovery plan, as there is an undeniable need for continued support post-treatment. Relapsing is a real possibility, and clients who struggle with addiction may also have mental health issues. When the two are combined, it makes the recovery effort that much more challenging.

Aftercare is dedicated to understanding a client’s mental state and needs, which can then assist in maintaining a sober lifestyle. Typically, this involves understanding triggers, developing coping mechanisms for cravings and stressors, understanding the dangers of a relapse, and preventing a lapse from escalating.

Does Transcend Provide Aftercare Programs?

Transcend understands the need for comprehensive and sustained aftercare provisions for those who are on the road to recovery. To this end, we, along with our sister company, The Heights Treatment, provide aftercare treatment at various levels.

First, there is a partial hospitalization program (PHP), which is a comprehensive outpatient treatment program suited to clients who are transitioning from a residential treatment program. The structure of this program is very convenient, as treatment is completed on a consistent schedule while clients reside in their homes or a sober living facility.

Next, there is the Intensive Outpatient Program (IOP) that also runs for several days weekly, and only requires a few hours daily. There are many addictions and mental health issues that clients struggle with that can be tackled with the various therapy options that fall under this program. Some of these include Cognitive Behavioral Therapy, Shame Resilience, Dialectical Behavior Therapy, etc.

Upon completion of other programs, such as PHP and IOP, there is a dedicated outpatient program (OP) for continued aftercare treatment. This program is geared towards a gradual reintegration into a healthy life for our clients. There are weekly individual therapy sessions, as well as a check-in group with peers.

Transcend and The Heights Treatment have created a workflow that allows clients to work through aftercare treatment with progressively greater independence. Of course, this is synonymous with different levels of accountability.

Additionally, upon completion of a sober living program, we offer our clients individualized case management to assist them. Therefore, they can keep meeting their goals, sticking to their prescriptions, remain connected to the Transcend community, etc.

How Long and How Frequently Will You Need Aftercare?

Transcend realizes there is no hard and fast rule where the duration of after care is concerned. Clients are often at different stages in the recovery process, and even those who are at similar stages don’t always have the same needs.

We prefer to assess each situation on an individual basis to determine the frequency and level of aftercare a patient needs. If we were to skip this step, our clients may feel like nothing more than another number to us. The Transcend way is to communicate and demonstrate to clients that we value them as individuals, and that we are committed to fostering their recovery in a personally appropriate manner.

As indicated above, the program is segmented into different levels that provide various balances between structure and independence. Frequency and engagement durations also vary here. For example, a plan with a more significant structure may require a six-hour session, five days per week. Alternatively, there may be another plan that only requires a couple of hours one to two days per week.

Recovery is not a straightforward process, and surely some form of support is needed to get through it successfully. Participating in an aftercare program increases the likelihood of remaining sober for life.
