Transcend Mentoring Program

Recovery mentoring is the key to helping our clients rebuild their lives for long-term success. Transcend mentoring programs help support clients in the transition from a community living setting to more independence with intentional support until they are ready to be autonomous. The strength of the program is helping our clients rebuild a healthy foundation and healthy recovery community.

What makes this program unique and successful?

Relapse Plan/Behavior Contract:

All clients meet with their mentor to complete a behavioral contract/relapse plan in order to participate in mentoring. Included in this contract will be a personal plan to support and sustain their sobriety such as seeking employment, attending 3-5 twelve-step meetings per week, finding a sponsor, enrolling in school, participating in recovery community events etc. This contract will also entail all requirements and contingencies if a relapse occurs while the resident is under mentorship. If the client is a young adult, parents are part of this plan and will be asked for their input and then sign off before the transition goes into effect.

Goal setting:

The mentor will establish three long-term goals and three short-term goals with the client. They will do a next step intake with client to work out action plans to achieve their goals. The mentor will continually work to support the client to achieve their goals and check in on a daily basis.

Weekly Mentor Visits:

Every week the mentor will meet with their client from 3-9 hours depending on the level of care. Together they will fill out an extensive report packet, which highlights client progress, 12-step meeting attendance, medication inventory, goal accountability, fears & concerns, as well as identifying any signals or red flags that the client may be slipping into old behavior. The packet comprehensively acts as an all-encompassing perspective into the daily life of the client and addresses immediately which areas need improvement. The packets are then turned into the Transcend Mentoring Director who reviews them and collaborates on challenging concerns.

Random Weekly Drug Testing:

The mentor will be randomly testing the client on a weekly basis. The lab we use for testing, tests for a panel of over 48 different substances, making it nearly impossible for anyone to fake a test result.

Daily Phone Calls:

Mentor will contact the client on a daily basis to check in. The client will update the mentor on progress, challenges, or difficulties relating to their goals and to sobriety. The Mentor will also contact parents and/or any other family members involved in the client’s recovery process.

Home Inspection & Med Count:

Mentor will conduct a home inspection weekly, checking for any signs of narcotics or alcohol as well as making sure the client is living in a tidy, healthy, and suitable environment. If the client takes medication, the mentor will also do a med count to be certain each prescribed medication is being taken correctly & responsibly.


Every client under mentorship is required to participate in a recovery community at least once a week. If the client has been through Transcend Sober Living, they are required to participate at least once a week for dinner, group or activity to preserve the sense of community & fellowship which is experienced within our houses. This gives the client an opportunity to work with others and be helpful to those who are newly sober and in the early stages of their recovery.

Supervision and Training

All mentors are trained on mental health issues as well as drug and alcohol related interventions. Mentors participate in supervision meetings with the Director of Mentoring on a weekly basis.
